Our forms may be downloaded from the appropriate section to the left, completed, and submitted the following ways:
· Save electronically or scan, then email to applications@ccthd.org (If choosing this method please use the “draw” feature to sign the application.)

Payments:
Payments methods accepted are cash, check, money order, or by our online payment system.
To use our online payment system, please select the appropriate Department from the dropdown then select the appropriate regulated facility type that correlates with the application being submitted.
**PLEASE Note: Applications are not considered complete until we have the following items: Application, Supporting documents (if applicable) and Payment. Applications will not be processed until they are complete.
